  3. Observed: Some floor tiles by restrooms, ceiling tiles in kitchen, walk-in refrigerator door and East end of dry storage area are in poor repair. Required: Facility is required to be in good repair.
  4. Observed: A few areas of walls in kitchen and dishwashing areas are not clean. Required: Must clean as necessary.
  5. Observed: Container holding ice scoop was not clean. Promptly replaced with a different container. Required: Ice scoop must be stored in a clean container.

    1. Observed: Raw chicken being stored on top of oysters. Corrected on site by moving oysters above raw chicken. Required: Raw animal products must be properly stored according to cooking temperatures and below all ready to eat foods.
    2. Observed: Several pieces of equipment found being stored not clean (knives, dicer). Corrected on site by moving all equipment to dish area to be washed. Required: All food contact equipment must be properly cleaned after use and before storage. Long term solution: managment will have regular meetings with staff to review date marking procedures and will have daily checks to ensure all PHFs are properly dated.
    3. Observed: Several potentially hazardous (PHFs) being stored with no date markings (cooked noodles, lasagna, cooked vegetables) indicating when they were made/opened. Reinspection is scheduled for 4/27/2017. Repeat priority violation. Required: All PHFs must be properly date marked when they are prepped/opened and discarded 6 days later.
    Nonpriority Violations Found: 3
    1. Observed: No chlorine sanitizing test strips were found for the dish machine. Required: Must have the correct sanitizing test stips for the both the quat sanitizer buckets and the chlorine test strips.
    2. Observed: Several PHFs in the cooling process that were not being cooled in a manner that would allow them to drop in temperature quickly. Corrected on site by placing items in walk-in freezer. Required: PHFs must be cooled in a manner that allows them to properly cool from 135F to 70F in 2 hours and from 70F to 41F in 4 hours.
    3. Observed: Single use containers being reused to store different food items. Corrected on site by discarding all reused single use items. Required: Single use equipment may only be used a single time and then they must be discarded.

    1. Observed spray bottle in metal bucket stored over canned juice. Requried: Store chemicals away from drinks & food items so they cannot contaminate them or food contact surfaces. Corrected by moving spray bottle to floor away from drinks-4/25/17.
    2. Observed iodine sanitizer at bar sink dispensing too strong at 50 ppm. Required: Manufacture's directions on label indicate 25 ppm iodine residual. Manager agreed to call service company. If used in between bar tender agrees to adjust by diluting.

    1. Observed tongs handing on oven door. Required: Store tongs on a clean equipment that is sanitized at least every 4 hours such as pan. Corrected by storing in pan. 4/25/17
    2. Observed ice scoop at waitress soda fountatin in hold sleeve that has rust at bottom. Required: Store scoop in a clean, protected location Corrected by removing holder and using small pan. 4/25/17
    3. Observed cutting boards (x4) that are scored. Required: Surface should be smooth & cleanable; resurface or replace. Green cutting board taken out of service.
    4. Observed debris & soil build up on dish racks. Required: Dish racks should be clean to sight and touch.
    5. Observed liquor bottles with open tips stored on 3 vat and hand washing sink exposed to splash from water. Required: Store bottles away from splash & contamination. Corrected-4/25/17
