  • Observed: There were some refrigerated, ready-to-eat, potentially hazardous food items that were not labeled with "use by" dates: cooked wings, pan of bbq beans and chunks of cooked meat in walk-in refrigerator. Promptly corrected

  • Observed: Outside dumpster has a broken lid and was being left open.
  • Observed: The pressure relief valve on water heater and another water line in this closet were leaking.
  • Observed: There is a hole in wall by mop sink and in front area by tappers.
  • Observed: One open drain line by glass doored refrigerator needs sealed, sign of use by drain flies.

    Facility was given permission to reopen, since the priority violations were substantially corrected. However, there are a still a few issues that need more attention or not able to be confirmed that they have been corrected without food being prepared at this time. A visit will be made on or about 8-1-2023 to verify continued correction and other issues have been addressed. I have attached a list of issues that need to be addressed: 1. Sides of stoves and deep fryer need cleaned better. 2. 5 drums of wood ashes need removed from parking lot. 3. Ashes and wood debris needs removed from parking lot. 4. Continue to take actions to minimize pests and remove dead ones. 5. Inside of back oven and hot hold unit need cleaned better. 6. Floor in walk-in refrigerator needs cleaned. 7. Make sure that policies and procedures for Time as a Control and Cleaning schedules are being implemented and monitored to ensure compliance.

    Facility's food permit was suspended due to risks found at time of inspection. Once the priority and the designated core violations have been corrected, staff should contact our office for a reinspection to see if permit can be reissued.

  • Observed: 1. An infestation of mice inside facility. 2. There were live cockroaches observed beside the mop sink.
  • Observed: There were bbq beans and burnt ends with a documented discard time of 3:00 at 3:15 they were 102 and 95 degrees, respectively and still on steam table. These were voluntarily discarded.
  • Observed: Prep refrigerator was broken and pans of potato salad and coleslaw were setting on trays of ice and were 58-60 degrees F. These were promptly discarded at my request.
  • Observed: None of the refrigerated, ready-to-eat, potentially hazardous food in walk-in refrigerator were labeled with an "use by" date: cooked ham, turkey, ribs, brisket, etc...
  • Observed: Dishes were being sanitized in a solution of 10 ppm chlorine in 3 vat sink. Must be 50-100 ppm chlorine.

  • Observed: The wood stacked outside had debris built up under it and piles of bark could provide harborage for pests.
  • Observed: There were 5 dead and decomposing mice on glue board traps: 4 in water heater room and 1 in dry storage area.
  • Observed: There were no hand soap at front hand sink. Promptly corrected
  • Observed: There were no paper towels at front hand sink. Corrected promptly by staff
  • Observed: There were holes in wall beside upright freezer and beside mop sink where pests were observed.
  • Observed: The floors in kitchen, prep area, water heater room, walk-in refrigerator, hallway to exit, in front of walk-in refrigerator, and especially under and behind equipment had significant buildup. Also 2 box fans had heavy accumulation of debris on them.
  • Observed: The inside of hot hold cabinet, several shelves (inside kitchen and smoking area), inside and sides of ovens, as well as deep fryers had heavy buildup.
  • Observed: The outside grease dump tank had over an inch of debris on it's top surface and was open. Staff scraped the debris and put into dumpster.
  • Observed: There was an old broken prep refrigerator on parking lot behind facility.
  • Observed: There were several stacks of foil pans that were being stored with the food contact surface facing upward.

    The following items still need to be worked on: 1. Some foods items that were on Time as a Control were not documented. Corrected prior to exit. These must be documented with time that they must be disposed of. 2. A few items in the hot hold cabinet were 130 degrees F. These must be held at 135 or above. 3. Floor in walk-in refrigerator and some areas of kitchen needed cleaned better. The other areas were much better.

    A reinspection will be made on or after 2-15-2023 to check for compliance.

  • Observed: Facility had lemon scented bleach for doing dishes. I sent staff to store to get unscented bleach and it arrived prior to facility opening.
  • Observed: The drain under 3 vat sink leaks and a bus tub is setting under it also hot water is shutoff at lavatory in men's restroom due to a leak.
  • Observed: There was evidence of mice in kitchen and room with water heater. These areas must be cleaned up and actions taken to minimize mice.
  • Observed: Some cooked meats had been reheated to 125 then placed into a hot hold cabinet that had just been turned on. This was promptly removed and reheated to 165 degrees. Unit 148 degrees at 11:15 AM.

    • Nonpriority Violations Found: 6
  • Observed: The floors in back room, kitchen and walk-in refrigerator were not clean.
  • Observed: Several food containers were not stored inverted.
  • Observed: Facility was using Time as a Control on some precooked items, but nothing was documented.
  • Observed: Out of paper towels in men's restroom and dispenser in women's restroom is not operational.
  • Observed: The front counter and some in kitchen were not clean.
  • Observed: One chubb of raw ground beef was being stored above cooked meat items. This was promptly relocated to bottom shelf at my request.

    Brandon was person in charge at time of reinspection. Facility has taken actions to minimize mice, but still needs to clean up area better. There was open mouse poison, that needs to be put into tamper resistant bait stations. Also 3 vat sink is leaking and has a container underneath to catch the wastewater. A reinspection will be made on or after 10-11-2022 to check these violations.
